Required Before Participation

Facility Waiver & Release Form

This Waiver & Release of Liability ("Agreement") is entered into between the participant and Steamers Crew Power Haus. By signing this form, the participant (or parent/guardian if under 18) acknowledges and agrees to the following terms:

Assumption of Risk

I understand that participation in sports, training, and recreational activities at Steamers Crew Power Haus involves inherent risks, including but not limited to physical exertion, collisions, falls, equipment use, and potential exposure to illness. I voluntarily assume all risks associated with participation.

Release of Liability

I release, discharge, and hold harmless Steamers Crew Power Haus, its staff, coaches, owners, affiliates, and partners from any and all claims, demands, or causes of action arising out of any injury, illness, or property damage sustained during or related to activities at the facility.

Medical Authorization

In the event of an emergency, I authorize Steamers Crew Power Haus staff to obtain medical treatment for myself or my child. I accept full responsibility for any resulting medical expenses.

Facility Rules & Conduct

I agree to follow all posted facility rules and staff instructions. I understand that violation of rules or unsafe behavior may result in removal from the facility without refund.

Waiver of Claims

I waive any right to bring legal action against Steamers Crew Power Haus for ordinary negligence. This waiver does not apply to claims of gross negligence or intentional misconduct.

By signing below, I confirm that I have read, understood, and voluntarily agree to the terms of this Waiver & Release of Liability.
